The origins of SMU lie in an agreement dated 12 March 1992 between the Government of Sikkim and the Manipal Education and Medical Group. The agreement concerned the development of educational and healthcare services in Sikkim, including medical education and a referral teaching hospital.
The Sikkim Legislature subsequently established the university through the Sikkim Manipal University of Health, Medical and Technological Sciences Act, 1995, identified as Sikkim Act No. 9 of 1995. The legislation created and incorporated the institution as a university in Sikkim.
