Application Call for Free Health Insurance Reimbursement
Sudurpashchim Province Government, Ministry of Internal Affairs and Law, Sudurpashchim Province, Dhangadhi, Kailali, Nepal, Notice regarding call for applications.
For Fiscal Year 2082/083, the Ministry of Internal Affairs and Law, Sudurpashchim Province, has included a free health insurance program for conflict victims, disappeared persons and martyr families in its approved annual programme.
Accordingly, those who meet all of the following conditions will receive reimbursement of the amount paid for health insurance from this Ministry’s programme:

Permanently residing within Sudurpashchim Province

Belonging to one of the following categories:
-
Conflict victims
-
Families of disappeared persons
-
Martyr families
-
Gen Z Movement martyr families
-
Injured persons of the Gen Z Movement
-
-
Enrolled in the health insurance programme operated by the Health Insurance Board, Government of Nepal
-
Having done health insurance for the current Fiscal Year 2082/083
Applications are therefore invited as follows.

Application (request letter)

Original receipt of health insurance payment and a copy of the health insurance identity card

Copy of Nepali citizenship certificate

Relationship-verifying document or relationship certificate

Documents that establish the identity as:

Conflict victim
-
Disappeared person’s family
-
Martyr / Gen Z Movement martyr family

Injured person of the Gen Z Movement

Copy of cheque clearly showing the bank account name and number
