Notice Regarding Submission of Examination Application Form for Four-Year Bachelor Level Fourth Year Supplementary Examination.

Tribhuvan University, Office of the Controller of Examinations (OCE), Balkhu, hereby notifies all students who appeared in the annual examinations of the Faculty of Humanities and Social Sciences, Faculty of Management, Faculty of Education, and Institute of Science and Technology (IoST) under the annual examination system in 2081 B.S.

As per the decision of the TU Academic Council to conduct the supplementary examination, students fulfilling the following criteria are informed to submit the examination application form as specified below.

1. Regarding the Supplementary Examination Application Form

1. Applicants must have been regular examinees of the Four-Year Bachelor Level Fourth Year Examination of 2081 B.S.

2. Applicants must have passed all subjects (including practical examinations) of the First, Second, and Third Years within the application period.

3. Only those students who failed or were absent in not more than two subjects in the Fourth Year Examination of 2081 B.S. are eligible to participate in this supplementary examination.

4. The supplementary examination shall be conducted under the same roll number used in the 2081 Fourth Year Examination. Therefore, candidates must mention the same roll number while filling the application form.

5. The application form must be filled online through the respective campus between 2082 Ashoj 26 to Kartik 14 by completing the process mentioned in points (1) to (4) above.

6. Students who were absent or failed in practical examinations and internal assessments must also fill the form. The concerned campuses/colleges must submit the marks of such subjects to the OCE, Balkhu or the respective Regional OCE within seven (7) days after the deadline of the application submission period.

7. Examination centers have been designated in one location per province. Examination centers for candidates from Kathmandu Valley and surrounding districts will be published later.

2. Examination Centers

1. Degree Campus, Biratnagar Examination Center:
   For all campuses and examinees under Koshi Province, and for all campuses of Siraha and Saptari districts.

2. Janjyoti Multiple Campus, Chandranigahpur, Rautahat Examination Center:
   For all campuses and examinees from Dhanusha, Mahottari, Sarlahi, Rautahat, Bara, Parsa, and Sindhuli districts.

3. Prithvi Narayan Campus, Pokhara Examination Center:
   For all campuses and examinees under Gandaki Province.

4. Bhairahawa Multiple Campus, Bhairahawa Examination Center:
   For all campuses and examinees from Rupandehi, Nawalparasi (East and West), Kapilvastu, Arghakhanchi, Gulmi, and Palpa districts.

5. Mahendra Multiple Campus, Nepalgunj Examination Center:
   For all campuses and examinees from Bardiya, Banke, Dang, and all campuses under Karnali Province.

6. Asian Management College, Dhangadhi Examination Center:
   For all campuses and examinees under Sudurpaschim Province.

7. Kathmandu Examination Center:
   For all campuses and examinees from Dolakha, Ramechhap, Sindhupalchok, Kavrepalanchok, Rasuwa, Nuwakot, Dhading, Makawanpur, Chitwan, and the districts within Kathmandu Valley. (Detailed center information will be published later.)

3. Regarding Application Form Submission and Fees

1. Campuses within Kathmandu Valley must submit the bank voucher along with the application forms to the TU OCE, Balkhu within two (2) days of application submission.
   Campuses outside the Valley must submit the bank voucher and application forms to their respective Regional OCEs within four (4) days, and the Regional OCEs must then forward them to the TU OCE, Balkhu.

2. While submitting the examination forms, the bank voucher must be verified at the Accounts Section of the concerned OCE on the next day after the application deadline, before submission to the respective section.

4. Examination Fees

• Per Subject Fee: Rs. 1000

• Application Form Fee: Rs. 100

• Examination Center Fee (per subject): Rs. 150

• Practical Examination Fee (per subject): Rs. 200

• Practical Examination Fee (for two or more subjects): Rs. 300

• Teaching Practice Fee: Rs. 350 (of which Rs. 50 shall be deposited in OCE, Balkhu; Rs. 150 in the Faculty of Education, Dean’s Office; and Rs. 150 in the concerned campus account)

• Field Work, Term Paper, and Project Work Fee: Rs. 300

5. Payment of Examination Fees

1. For Campuses within Kathmandu Valley:
   Examination-related fees must be deposited in either of the following accounts:

   • Global IME Bank Ltd., TU Kirtipur/Balkhu Premises – Account No. 0501010000500

   • Nepal Bank Ltd. – Account No. 04500100191060000001
     The deposit voucher must be submitted along with the examination application forms in the name of the OCE.

2. For Regional OCE Pokhara: Fees must be deposited in Nepal Bank Ltd., Bagar Branch – Account No. 01100101101344000001 in the name of TU Regional Office, Pokhara.

3. For Regional OCE Biratnagar: Fees must be deposited in Nepal Bank Ltd., Bazar Adda Branch – Account No. 10000100922120000001 in the name of TU Regional Office, Biratnagar.

4. For Regional OCE Nepalgunj: Fees must be deposited in Global IME Bank Ltd., Tribhuvan Chowk, Banke – Account No. 2101010000076 in the name of TU Regional Office, Nepalgunj.

5. For Regional OCE Attariya: 
   Fees must be deposited in either of the following accounts:

   • Global IME Bank Ltd., TU Balkhu Premises – Account No. 0501010000500

   • Nepal Bank Ltd. – Account No. 04500100191060000001
     The deposit voucher must be submitted along with the examination application forms in the name of the OCE.
